Office Policies:

• 25 dollars will be charged for returned checks
• Charge for missed Appointments: 25 dollar charge for failing to cancel a regular appointment more than 24hours in advance ($50 for physicals missed)
• This fee will be waived in emergencies.
• These fees are not covered by insurance including HMO insurance.
• It is my policy not to overbook my schedule in order to try to avoid longs waits in the waiting room. If you miss an appointment with out notice, it is only fair that you pay for the overhead costs that the time you reserved.
